About:
The OPHP Preventive Block Grant (PBG) is provided by The Preventive Health and Health Services (PHHS) Block Grant Program at the Centers for Disease Control and Prevention (CDC). OPHP then provides this grant money to local public health agencies, throughout the State of Colorado, as the OPHP Preventive Block Grant (PBG). This grant is designed to support local public health partnerships and planning efforts. This funding cycle runs October 1st - September 30th (Federal Fiscal Timeline) each year.
Note - expenses not allowed:
- In-patient services
- Construction purposes
- Equipment purchases over $5,000 (unless pre-approved)
- Supplanting loss of state funds
- Matches in order to secure other federal grants
- Cash payments (includes gift cards, stipends, vouchers)
- Food/beverage purchases

CURRENT CYCLE - PBG FFY2022-23 Deliverables:
Budgets & Work plans
- LPHAs will submit a total of four (4) work plans and budgets throughout the year.
- Initial within 30 days of contract execution
- April with mid-point report
- July 14
- August with final report
Reports
- Mid-year Report, 4/28/2023 - OPHP PBG FY22-23 Mid-year Report Form
- Spend Down progress due by 08/15/2023, if unable to spend funds.
- Final Report, 8/31/2023
Mandatory Meetings
- Two mandatory meetings:
- Kick-off; at least one person from the LPHA must attend
- One more during the year.
- OPHP sets meetings in January, April, and July; at least one person from the LPHA must attend a Quarterly meeting.
