About:
The OPHP Preventive Block Grant (PBG) is provided by The Preventive Health and Health Services (PHHS) Block Grant Program at the Centers for Disease Control and Prevention (CDC). OPHP then provides this grant money to local public health agencies, throughout the State of Colorado, as the OPHP Preventive Block Grant (PBG). This grant is designed to support local public health partnerships and planning efforts. This funding cycle runs October 1st - September 30th (Federal Fiscal Timeline) each year.

Deliverables
- Budgets & Work plans (Three of each total)
- Initial within 30 days of Purchase Order Effective Date (Budget time period is 10/1/2023 - 9/30/2024)
OPHP Document New or Change Revision Request Submission Form - April 25, 2025 with mid-point report
- August 29, 2025 with final report
- Initial within 30 days of Purchase Order Effective Date (Budget time period is 10/1/2023 - 9/30/2024)
Reports
Mid-year Report due 4/25/2025
Final Report due 8/29/2025
Meetings (Two total, at least one person from the LPHA must attend)
- Kick-off - Thursday, November 7, 2024 at 12 pm MST (invitation to be sent)
- One Quarterly Meeting
- OPHP will host meetings on the third Wednesday of January, April, and July in 2025, from 11:00 am - 12:00 pm; at least one person from the LPHA must attend a Quarterly meeting.
- LPHA will share a 3 minute presentation of their project at a Quarterly meeting.
