2025-2029 State Health Improvement Plan 

Coinciding with the upcoming release of the 2025-2029 State Health Improvement Plan, OPHP will be hosting an informational SHIP Lunch & Learn webinar series. During this webinar series, subject matter experts will join us to talk about the eight priority areas, including the goals and strategies outlined in the SHIP. We’ll also have partners from local public health agencies, CALPHO, and the Colorado School of Public Health join various sessions to talk about work that they are doing in alignment with these priorities. Every five years, the Colorado Department of Public Health and Environment collaborates with partners across the state to assess the status of Colorado's public and environmental health and identify health and environment priorities for the next five years. This work results in a state health assessment (SHA) and state health improvement plan (SHIP.) Both of these documents are required by the 2008 Public Health Act and the national Public Health Accreditation Board. CDPHE and partners have completed the 2023 State Health Assessment and are currently in the process of developing the 2025-2029 State Health Improvement Plan. In accordance with the Colorado Health Assessment and Planning (CHAPS) process, our Office also developed the following 2023 Capacity Assessment.
