Phase 5: Prioritize issues
Step 1: Identify a list of issues to consider based on assessment results
Step 2: Design a prioritization process
Step 3: Develop a summary presentation of each issue
Step 4: Facilitate a prioritization process to determine PHIP focus area(s)

Phase 6: Public Health Improvement Plan (PHIP)
Step 1: Develop a planning process that includes scope and needed resources
